Registered Nurse (RN) | Home Health
Location: Grand Rapids, MI
Agency: Pyramid Consulting, Inc.
Pay: $65 to $70 per hour
Shift Information: Days - 5 days x 8 hours
Start Date: 10/5/2026

About the Position

Key Responsibilities

  • Provide skilled nursing care to patients in their homes.
  • Perform comprehensive patient assessments and develop/implement individualized care plans.
  • Monitor patient conditions and communicate changes to the appropriate clinical team.
  • Complete accurate and timely clinical documentation.
  • Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
  • Educate patients and families regarding medications, treatment plans, and disease management.
  • Follow home-health clinical, regulatory, and safety requirements.
  • Complete required OASIS assessments and documentation .
  • Maintain compliance with agency policies and applicable healthcare standards.

Requirements

Requirements

  • Active Registered Nurse (RN) license.
  • Previous home-health nursing experience .
  • Strong OASIS experience.
  • SHA experience required/preferred based on client requirements.
  • Excellent clinical assessment and documentation skills.
  • Strong communication and patient-care skills.
  • Ability to work independently in a home-health environment.
